In Vitro Fertilization

In vitro fertilization: assisted reproduction technique This infertility treatment is the third step in the assisted reproduction techniques ladder. This treatment brings the best pregnancy rate. Also called in vitro insemination, this technique is performed in assisted reproduction centers due to its high complexity. Indications for in vitro fertilization Couples that didn’t succeed with 4 intrauterine insemination cycles. Woman with obstructed Fallopian tubes. Moderate or severe decrease in the number and motility of sperm. When men have had a vasectomy in the past. In vitro fertilization consists in impregnating the eggs with the sperm in the lab, in order to obtain embryo that will after, be placed inside the uterus. This technique exists since 1978 when, Louise Brown, first baby to be conceived by this technique, was born in England. Through all these years, this method has improved and is used nowadays in thousands of clinics around the world. When first starting with this technique, the question of affecting the development or growth of the child was a big issue. After some decades, today we know that the use of this technique does not affect the child. 5 million babies have been born through in vitro fertilization since 1978. The objective of this technique is to ensure the impregnation of the egg by the sperm, correcting any factor causing infertility both in the male or female companion. In this technique, both male companion or donor semen can be used. In vitro fertilization procedure The in vitro fertilization procedure has 5 steps: Ovarian stimulation It consists in stimulating the ovaries with oral or injected medicines, making them produce one or two eggs. This stimulation lasts between 8 and 14 days. It is very important to have ultrasounds every 3 or 4 days to control – adjunting the dosis – the number and growth of eggs that are being developed. When it is observed through ultrasound that the folicules reached the right size, this indicates the prefect moment to make them ovulate through an injection. The doctor will indicate the exact moment when the medicine to ovulate should be applied, having the extraction of the eggs done 36 hours before the ovulation. This control should be done by specialists, through vaginal ultrasound, in order to avoid any complication such as a multiple pregnancy or ovaric hyperstimulation syndrome. Show more Semen preparation in the lab The day of the eggs extraction, just a few hours before the procedure, the semen sample is prepared. This sample is capacitated in the andrology lab to select the best sperm. Once selected, they are prepared to impregnate the egg. Ovarian punction: egg extraction Before the ovulation happens, the eggs are extracted and placed in culture media inside special incubators in the embriology lab. The extraction is performed with a needle that is introduced in the vagina with the help of an ultrasound. This step is known as ovarian punction. This procedure lasts about 20 minutes and a light sedative is used in order to ease the pain. It is not necessary any incision so, there will be no scar. After the punction, the patient remains at ease for some hours, waiting for the effects of the sedative to go away. The patient can go home righ after. Insemination of the eggs and embryo culture Once we have the sperm and eggs, the impregnation is done in the embriology lab. This can be done in 2 ways: Conventional in vitro fertilization: in a drop of culture media, some capacitated sperm is placed together with the egg. This way, the Intracytoplasmic sperm injection joins the egg to fertilize it. Sperm Intracytoplasmic sperm injection: with the help of pipettes and an special microscope, one sperm is chosen and injected directly inside the egg After, the inseminated eggs are placed inside incubators and left in the culture during 19 hours, when then they are observed to know if they are fertilized or not. Embryo transfer After 3 or 5 days in culture media, the embryo are transfered inside the uterus. This is a procedure that does not need anesthesia, lasts a couple of minutes and the disconfort is similar to that of a Pap test. 15 days later, a pregnancy test is done.

Espera y Prueba de Embarazo

Espera y prueba de embarazoUna vez terminado el tratamiento, no queda otra cosa más que esperar a la prueba de embarazo. Este tiempo es, en muchos casos, un momento de nerviosismo por conocer el resultado pero, debemos estar seguros y tranquilos ya que nos hemos puesto en manos de un centro especialista como Centro FiVER y nuestro doctor nos ha explicado todos los detalles del tratamiento. Todos hemos puestotoda nuestra energía en que llegue su “Bebé FiVER”.Esta fase del tratamiento es conocida como “Beta-espera”El nombre de “Beta-espera” se debe al tiempo que tenemos que esperar, aproximadamente unos 14 días, para poder realizar la prueba de embarazo, conocida como medición de la hormona del embarazo – beta HCG. Esta prueba de embarazo no se trata de la tradicional que sepuede solicitar en cualquier farmacia, sino que se trata de una prueba de embarazo en sangre donde se miden niveles hormonales.Durante las aproximadamente dos semanas de espera, es importante no alterar nuestros ritmos de vida y mantenernos tranquilos y distraídos.Durante este tiempo, a pesar del nerviosismo y las ganas de saber qué ocurrirá, se desaconseja acudir a pruebas de embarazo tradicionales que, en estos casos, pueden dar resultados erróneos. To perform this technique, it is necessary to have an in vitro fertilization in an assisted reproduction center. This center should have an egg donation program. If you are having your menstrual cycle, it’s okay to assist to the appointment and perform the studies as, in many cases, it is indicated in order to reduce the diagnosis duration and beginning of the treatment.The basic infertility studies are:For women:Hormonal study: performed during the first days of the cycle, when the folicules grow.Hysterosalpingography (fallopian tube examination):It is a study that measures the tubes permeability (of the fallopian tubes and uterus). This study is very important and will mostly determine the complexity of the needed treatment.This relatively simple and quick study is done with X rays equipment and a contrast media that will go through the Fallopian tubes. Through the image obtained (radiography), the specialist will determine the tubes permeability.For men:Hormonal study: blood analysis that measures the hormones levels. Sperm study / Spermiogram:This is the main study for men, must be done in an specialized center with the proper equipment that will measure the behavior and quality of sperm.After having the seminal sample, for which we need to book a previous appointment, the sample will be processed and studied in the assisted reproduction lab to obtain the results some hours later.If the sample will not be taken in a specialized center, it will be necessary to receive proper indications from the specialized team of the reproduction lab, about the sample recolection and transportation.For the couple (when necessary):Stress levels measurement:Through this study, other infertility causes – non medical related – are analyzed.It consists of and interview and application of simple tests to both members of the couple before starting the treatment. Generalidades de Infertilidad

Infertilidad: problema común y generalizado que ha ido en aumento La infertilidad es un problema muy común que afecta a millones de personas. Se estima que 2 de cada 10 parejas en edad reproductiva padecen infertilidad. Esto supone que, a nivel mundial, hay entre 50 a 80 millones de personas con este problema. La infertilidad no respeta raza, estatus social, religión o nacionalidad. México no es la excepción. Se estima que en nuestro país afecta a más de un millón de personas. La infertilidad ha ido en aumento en los últimos tiempos debido a factores ambientales como: contaminación, cambios en el estilo de vida que producen estrés, así como el deseo de postergar el formar una familia. Anteriormente, se buscaba el embarazo entre los 20 y 30 años de edad de la mujer. Hoy en día, un gran número de mujeres desea terminar sus estudios y desarrollarse profesionalmente, por ello empiezan a pensar en el embarazo después de los 35 años. Es a esta edad precisamente, que la mujer comienza el declive de su reserva de óvulos hasta llegar a la menopausia. Así, la probabilidad de lograr un embarazo disminuye a partir de los 35 años de edad. Mostrar más La infertilidad está considerada como una enfermedad El ser humano tiene arraigado el instinto de procreación y de protección de la descendencia desde sus ancestros. El hecho de no poder tener hijos tiene un impacto emocional bastante importante para algunas parejas que sufren infertilidad; pudiendo llegar a afectar su estado de salud, las relaciones sociales con su pareja, su familia o su trabajo. Por lo anterior, la Organización Mundial de la Salud considera la Infertilidad como una enfermedad. Debido a su impacto y al número de personas que la padecen, en algunos países llega a representar un importante problema de salud. Definición de Infertilidad: ¿Cuándo se considera que hay infertilidad? La organización mundial de la salud y las distintas sociedades de especialistas de reproducción coinciden en definir la infertilidad como: “incapacidad de lograr un embarazo clínico después de 12 meses o más de relaciones sexuales regulares sin protección”. Consideraciones importantes: Se consideran 12 meses ya que, la tasa de embarazo en parejas que no tienen ningún problema de infertilidad es del 10 al 20% por mes. Según varios estudios realizados con gran número de parejas, el 100% de las parejas sin problemas de infertilidad logran el embarazo en los 12 primeros meses. Mostrar más Efecto de la edad en la fertilidad de la mujer Desde que nace, la mujer tiene un número determinado de óvulos que, mes con mes se van desarrollando y preparando para ser ovulados y poder, en su caso, ser fecundados. Conforme pasan los años y con cada ciclo menstrual, la reserva de óvulos en los ovarios va disminuyendo, haciéndose más evidente a partir de los 35 años, momento en el que empieza a declinar hasta llegar a agotarse y empezar la menopausia. Las células precursoras de los óvulos tienen la peculiaridad, a diferencia de los espermatozoides en el hombre, de no regenerarse ni dividirse produciendo nuevos óvulos. Así, una mujer de 39 años tendrá óvulos también de 39 años de edad. Por esto, los hombres pueden tener hijos a edades más avanzadas que la mujer. A mayor edad de la mujer, mayor probabilidad de que sus óvulos tengan algún problema causado por esa edad, causando así, un problema de infertilidad. Estos problemas ocurren principalmente en mujeres mayores de 40 años.

Logramos el Embarazo

Logramos el embarazo¡Felicidades! ¡Lo hemos logrado! Todo el camino recorrido ha merecido la pena. Ya hemos dado el paso más importante para tener a nuestro “Bebé FiVER”: estar embarazada.Una vez que se ha acudido a consulta y el doctor ha dado la noticia, ya sólo quedan 9 meses de espera y cuidado hasta ver la carita de su bebé. Durante estos 9 meses deberá tener los cuidados de cualquier embarazo. Su ginecólogo habitual lo sabe y, tanto su especialista FiVER como él, se lo harán saber. De cualquier modo, el doctor en Centro FiVER realizará un informe para que su ginecólogo esté perfectamente informado. Sólo debe disfrutar de los próximos 9 meses que le esperan llenos de cambios y, para ello, a continuación le damos algunas recomendaciones.Es importante cuidar la dieta. En muchas ocasiones, los primeros días se puede sentir angustia y malestar, algo común en embarazadas, por lo que una dieta ligera le ayudará a sobrellevar esta situación. Además, durante todo el embarazo es importante mantenerse bien nutrido y alimentado sin aumentar excesivamente de peso. Por este motivo, puede acudir a su nutrióloga en Centro FiVER para que le apoye durante todo este caminopara no aumentar en exceso de peso, y posteriormente, mejorar la recuperación.Algunas otras recomendaciones a tener en cuenta son las siguientes:Consumir como mínimo 2 litros de agua por día y evitar el consumo de cafeína (café, té, refrescos de cola,…)No realizar ayunos prolongados.Visitar una vez al mes al ginecólogo para revisar su estado de salud y el de su bebé.Realizar los análisis clínicos correspondientes.Existe la posibilidad de hacer estudios genéticos del bebé durante el embarazo.Puede realizar un Ultrasonido de 4ª Dimensión (4D) que permite saber la buena formación y el buen estado del bebé. Además, veremos una imagen muy realista, como si de una foto se tratara (estudio que puedes realizar en Centro FiVER) de nuestro “Bebé FiVER”.Muy recomendable para la preparación del momento de dar a luz es la Gimnasia Preparto.Los días previos al parto no se debe olvidar todo lo que se puede necesitar durante la estancia en la clínica. Debe tenerse en cuenta que nunca sabemos en qué momento el bebé va a decidir que es el momento de salir y todo debe estar listo, tanto para la mamá (ropa y artículos personales) como para el bebé (ropita, pañales, biberones,…).Además, se debe ir preparando todo lo necesario en casa para la llegada del bebé, así como la organización del “Baby Shower” para darle la bienvenida y hacer su presentación oficial. Andrology Services

Andrology lab This is where the semen is received and prepared for study to be used later for assisted reproduction treatments. This lab is key in an assisted reproduction center as it is only used to handle the semen sample. The difference with conventional labs where other samples are handled: blood, fluids, bacteria culture, etc. is that the andrology lab has the environment that protects the sperm from any contamination or affectation. It is essential to go to an andrology lab for the sperm tests, avoiding the conventional labs. In FiVER, we have an andrology lab fully equipped for the assisted reproduction treatments and for the cryopreservation of the sperm. Sperm studies The sperm studies are key for the infertility diagnosis. As 40% of cases are due to masculine causes, these sperm studies become essential. Sperm in urinalysis The urinalysis study allows us to detect if retrograde ejaculation exists. This problem appears when, at the time of ejaculating, the semen goes to the urine instead of going out. This option is also used for women with no male companion that wish to have a baby. The semen bank is formed by young healthy samples that have been through strict medical studies, lab, psicological and genetic studies so any hereditary, infectious or chromosomal disease is discarded.
