Empezar el Tratamiento de RA

Empezar el tratamiento de RALo más importante para todo el equipo de Centro FiVER es que juntos alcancemos su objetivo: tener su “Bebé FiVER”. Para ello, ya hemos dado los pasos adecuados: buscar una clínica especializada en fertilidad, Centro FiVER;asistir a consulta y,realizar los estudios necesarios para tener un diagnóstico certero y saber cuál es el tratamiento que mejor se adapta a nuestras necesidades. Ya sólo nos falta un último paso en este camino que recorreremos juntos: el tratamiento de reproducción. El tratamiento en Centro FiVERTodos los tratamientos de reproducción de Centro FiVER se realizan en nuestras instalaciones en Veracruz sin necesidad de tener que desplazarnos a ningún otro lugar. Cualquier caso puede ser tratado en Centro FiVER. Esta es la única manera de asegurar los mejores resultados a nuestros pacientes y la cercanía a su especialista FiVER. Además, todo el personal de Centro FiVER, especialmente el equipo de enfermería, le apoyará en todo lo que usted necesite durante su tratamiento.Comienzo del tratamientoLos tratamientos de reproducción por lo general tienen en comúnuna serie de pasos que podemos conocer previamente:- EstimulaciónTodos los tratamientos comienzan con un periodo inicial conocido como estimulación de la mujer. En esta fase inicial, antes del procedimiento, el doctor le prescribirá medicamento para estimular sus ovarios.Durante esta fase, el especialista FiVER le agendará continuas visitas en las que revisará su estado y la evolución del tratamiento.En determinados casos, el varón también recibe un tratamiento para la mejora de los resultados.- Procedimientos médicos y de laboratorioUna vez terminada la estimulación, su doctor FiVER le comunicará que ya está lista para el procedimiento y fijarán la fecha del mismo. En esta fase es muy importante la coordinación con el laboratorio de reproducción asistida, por este motivo, el hecho de que tanto el laboratorio de reproducción asistida como que el doctor se encuentren en el mismo lugar, facilitará la comunicación. Además, evitará desplazamientos para el paciente y mejorará los resultados del tratamiento.Durante los procedimientos médicos y de laboratorio, además de la coordinación del doctor con el laboratorio, es muy importante el papel de la pareja ya que, ambos deben someterse a diferentes procedimientos médicos para llevar a cabo el tratamiento.Los procedimientos médicos se realizan en las instalaciones de Centro FiVER que también posee un laboratorio de reproducción asistida de última generación donde se llevan a cabo los complejos procesos de fertilización (FIV/ICSI) y preparación de las muestras.En el laboratorio, situado junto a la sala de procedimientos, se realizan los diferentes métodos de reproducción asistida para que su doctor FiVER realice el tratamiento para el que se ha estado preparando durante todo este tiempo.En este momento del tratamiento es imprescindible encontrarse relajado y concentrado en lo más importante: lograr el embarazo. Para ello, incluido en el tratamiento, contamos con el asesoramiento de especialistas que nos apoyan con diferentes técnicas de relajación.- Beta-espera y resultado (prueba de embarazo)Es el tiempo desde que se termina el tratamiento, hasta que se recibe el resultado de la prueba de embarazo, algo que podremos ver a continuación.

In Vitro Fertilization

In vitro fertilization: assisted reproduction technique This infertility treatment is the third step in the assisted reproduction techniques ladder. This treatment brings the best pregnancy rate. Also called in vitro insemination, this technique is performed in assisted reproduction centers due to its high complexity. Indications for in vitro fertilization Couples that didn’t succeed with 4 intrauterine insemination cycles. Woman with obstructed Fallopian tubes. Moderate or severe decrease in the number and motility of sperm. When men have had a vasectomy in the past. In vitro fertilization consists in impregnating the eggs with the sperm in the lab, in order to obtain embryo that will after, be placed inside the uterus. This technique exists since 1978 when, Louise Brown, first baby to be conceived by this technique, was born in England. Through all these years, this method has improved and is used nowadays in thousands of clinics around the world. When first starting with this technique, the question of affecting the development or growth of the child was a big issue. After some decades, today we know that the use of this technique does not affect the child. 5 million babies have been born through in vitro fertilization since 1978. The objective of this technique is to ensure the impregnation of the egg by the sperm, correcting any factor causing infertility both in the male or female companion. In this technique, both male companion or donor semen can be used. In vitro fertilization procedure The in vitro fertilization procedure has 5 steps: Ovarian stimulation It consists in stimulating the ovaries with oral or injected medicines, making them produce one or two eggs. This stimulation lasts between 8 and 14 days. It is very important to have ultrasounds every 3 or 4 days to control – adjunting the dosis – the number and growth of eggs that are being developed. When it is observed through ultrasound that the folicules reached the right size, this indicates the prefect moment to make them ovulate through an injection. The doctor will indicate the exact moment when the medicine to ovulate should be applied, having the extraction of the eggs done 36 hours before the ovulation. This control should be done by specialists, through vaginal ultrasound, in order to avoid any complication such as a multiple pregnancy or ovaric hyperstimulation syndrome. Show more Semen preparation in the lab The day of the eggs extraction, just a few hours before the procedure, the semen sample is prepared. This sample is capacitated in the andrology lab to select the best sperm. Once selected, they are prepared to impregnate the egg. Ovarian punction: egg extraction Before the ovulation happens, the eggs are extracted and placed in culture media inside special incubators in the embriology lab. The extraction is performed with a needle that is introduced in the vagina with the help of an ultrasound. This step is known as ovarian punction. This procedure lasts about 20 minutes and a light sedative is used in order to ease the pain. It is not necessary any incision so, there will be no scar. After the punction, the patient remains at ease for some hours, waiting for the effects of the sedative to go away. The patient can go home righ after. Insemination of the eggs and embryo culture Once we have the sperm and eggs, the impregnation is done in the embriology lab. This can be done in 2 ways: Conventional in vitro fertilization: in a drop of culture media, some capacitated sperm is placed together with the egg. This way, the Intracytoplasmic sperm injection joins the egg to fertilize it. Sperm Intracytoplasmic sperm injection: with the help of pipettes and an special microscope, one sperm is chosen and injected directly inside the egg After, the inseminated eggs are placed inside incubators and left in the culture during 19 hours, when then they are observed to know if they are fertilized or not. Embryo transfer After 3 or 5 days in culture media, the embryo are transfered inside the uterus. This is a procedure that does not need anesthesia, lasts a couple of minutes and the disconfort is similar to that of a Pap test. 15 days later, a pregnancy test is done.

Request your first appointment Before requesting this first appointment, you need to consider the following: There will be a long process: since you will first go to your check up appointment to have your diagnosis, until you have your treatment to get pregnant. It is very important that you plan this process carefully as any lost time can be key for a woman older than 35 years old. In FiVER, there are several specialists that will treat you as soon as possible so you don’t have any time to lose. Any moment of the menstrual cycle is fine for the first appointment. In fact, it would be better if the woman is having her period at that moment. For the patients that live outside Veracruz, we recommend you to come to FiVER once to have the first appointment as well as the basic infertility studies. We ask you to contact our patient support staff that will book you everything in one day. In some cases, some studies will be necessary but won’t be possible to have them the same day. If you haven’t done any test in the past, we recommend to the male partner to have from 3 to 5 days of sexual abstinence. You can have breakfast before arriving to your appointment. I will only be necessary for the patients having the tests on the same day to arrive with an empty stomach. Our staff will give you all the information if this is your case. Your first appointment will be a medical valoration with one of our infertility specialists as well as a stress and anxiety valoration with our patient support team. When you arrive to FiVER, our team will give you our data protection notice and will open a file with your data before seeing our specialist. Appointment: our specialist will listen closely to your case and will create a trust relation with you. This appointment will last for aproximately 1 hour and you will have: Check up of previous studies Creation of your medical history Physical exploration and gynecologic review: our specialist will review the cervix and vagina. This exploration may not be necessary in some cases. Vaginal ultrasound: this is the only tool that allows us to see the ovaries and uterus. Infertility studies appointment: in case you need more studies than the ones already handed over to the doctor, he will explain you all about them, how to prepare yourself and the time they will take. o Clear explanation of the infertility problem and answer to all your questions. Stress and anxiety evaluation: this evaluation lasts around one hour and is done in a different appointment the same day (for the patients living away from Veracruz) or on a different date. This evaluation will be done by our psycologist who will evaluate the stress and anxiety levels of the couple (or single person) with a large conversation. This will create a trust relationship to have the emotional needed support through all the treatment process. You will have also lots of mixed up information with all your previous results, studies, treatments and information taken from internet and other people. It is normal. We recommend you the following when attending your first appointment: Bring all the previous studies you have: separating them by person (you and your couple) and ordering them by date, from the oldest to the latest. Gather all the prescriptions you may have, separating them the same way as above Have a little calendar notebook to: Write the date of the first moment of the period (first stain) and the last day. Write any blood loss you may have between your periods and any relevant information. Write all your doubts along the way Read all the information in our section If you are having your period at the moment of the appointment it’s okay, it will be even better for the doctor that you are starting your period so he can gather important data. All specialists are used to do the check ups at this stage. The ultrasound and vagina check up last only few minutes.
