La mayoría de los casos de infertilidad masculina y femenina se pueden diagnosticar con los estudios básicos de infertilidad Al realizar los estudios básicos de infertilidad se puede llegar a un diagnóstico correcto en más del 90% de los casos. Para esto hay que estudiar a ambos integrantes de la pareja realizando una historia clínica y exploración física completa y realizar los estudios básicos de infertilidad que sean necesarios para cada caso en particular. Debido a que algunos de estos estudios se deben de realizar en ciertos días del ciclo menstrual de la mujer es muy importante programar adecuadamente todos los estudios para poder llegar a un diagnóstico en menos de 2 meses y no perder tiempo innecesario. Cada pareja y cada caso de infertilidad es diferente por lo que los estudios son individualizados, los estudios que necesita una pareja pueden no ser necesarios en otra. Estudios básicos de la pareja infértil Estudio de los ovarios y la ovulación: estudio hormonal, ultrasonido vaginal Estudio de las trompas: histerosalpingografía, laparoscopia. Estudio del varón: Espermiograma diagnóstico Estudio del útero: ultrasonido vaginal, histerosonografía, histeroscopia, laparosocopía Estudio de los ovarios y la ovulación Los estudios de fertilidad en la mujer están encaminados a investigar cómo están funcionando los ovarios, la reserva de óvulos que tienen y que suceda correctamente la ovulación. El estudio del funcionamiento ovárico se realiza de la siguiente manera: Valoración del ciclo menstrual: si los ciclos son regulares (regla cada mes) o no. Valoración de los ovarios por ultrasonido vaginal: se realiza conteo de folículos en los ovarios (imágenes obscuras vistas dentro del ovario donde se encuentran los óvulos) Estudios hormonales en sangre: perfil hormonal femenino, hormona antimülleriana. De acuerdo a la regularidad de los ciclos menstruales y la valoración por ultrasonido se decidirá si es necesario realizar el estudio hormonal o en su caso la determinación de hormona antimülleriana. En la mayoría de los casos suele bastar con un perfil hormonal femenino realizado entre el 2-4 día del periodo menstrual. Mostrar más El estudio por ultrasonido es muy importante, este debe de ser por vía vaginal ya que es más preciso y debe de realizarlo una persona experta. Al contar los folículos que hay en cada ovario se puede valorar la reserva de óvulos, ya que dentro de cada folículo se “supone” que hay un óvulo. Se dice se “supone” porque puede haber folículos vacíos que no contienen óvulos en su interior. Un conteo menor de 6 folículos en los primeros días del ciclo en ambos ovarios se puede considerar como una baja reserva ovárica aunque hay que tomar en cuenta la edad ya que no es lo mismo 6 folículos en pacientes de 41 años, que en pacientes de 34 años. Además del conteo de los folículos con el ultrasonido también se busca descartar la presencia de quistes u otras anomalías en los ovarios Estudio de las Trompas de Falopio Es necesario confirmar que las trompas de Falopio estén permeables (destapadas) para esto se realiza el estudio de la histerosalpingografía el cual consiste en la introducción de un medio de contraste a través del cérvix, en la cavidad uterina y posterior a esto, realización de una radiografía de pelvis para valorar como se rellenó la cavidad uterina y como es su paso a través de las trompas y al exterior de las mismas. Este estudio puede ser doloroso por lo que se deben de seguir algunas indicaciones antes de realizarlo. No en todos los casos es necesario realizar la hiserosalpingografía y va a depender del resultado del estudio de semen ya que en casos de factor masculino en donde hay pocos espermatozoides el especialista valorará si es necesario o no realizar este estudio. Estudio del varón Para estudiar al varón se realiza el estudio de semen llamado espermiograma diagnóstico, este estudio debe de realizarse en centros de reproducción asistida que tengan la infraestructura y la experiencia en este tipo de estudios. Se deben de utilizar los parámetros establecidos por la Organización Mundial de la Salud en cuanto a la cantidad, movilidad y formas normales además de otros datos como la presencia de células que sugieran infección entre otros. Mostrar más Es importante aclarar que en el varón la producción de espermatozoides se produce en 3 meses, y estos se renuevan constantemente durante toda su vida. Por esta razón la cantidad y movilidad de los espermatozoides pueden variar de un momento a otro y en ocasiones es necesario repetir el estudio de semen para corroborar el diagnóstico. El ejemplo más claro es en casos de que no aparezcan espermatozoides en el estudio es necesario repetirlo en 2 semanas o hasta 3 meses para poder afirmar la ausencia de ellos. En los casos en los que hay alteración severa de los espermatozoides se recomienda solicitar estudios de cariotipo (cromosomas) del varón y en otras ocasiones estudios de fragmentación del mismo espermatozoide. Cuando no hay espermatozoides se le llama azoospermia y es necesario realizar otros estudios como determinaciones hormonales, descartar infecciones y otros factores para valorar si se realiza una biopsia de testículo. Mostrar más La biopsia de testículo consiste en tomar una muestra de tejido testicular y confirmar la presencia de espermatozoides. En los casos de azoospermia obstructiva no aparecen espermatozoides en el semen pero si en el testículo con lo que se puede realizar una fertilización in vitro con inyección intracitoplasmática de espermatozoides (ICSI). Estudio del útero El estudio del útero se realiza por medio del ultrasonido vaginal en donde se pueden detectar datos que sugieren alguna anormalidad o nos hagan sospechar de alguna anomalía uterina. Cuando hay sospecha de alguna anormalidad se suelen solicitar otros estudios como: Histerosonografía: consiste en introducir solución salina dentro del útero y valorar por medio del ultrasonido si hay imágenes anormales en su interior. Este procedimiento puede realizarse en el mismo consultorio. Histerosalpingografía: consiste en introducir medio de contraste dentro del útero y realizar una radiografía lo que nos permite valorar el contorno completo del útero y detectar imágenes anormales.
